This is a group project and I am only responsible for sections 1 – 3 on the direction sheet which is highlighted in yellow. The proposed program is a Gun Violence Prevention program targeting youth ages 13 to 21 in the West Philadelphia Pennsylvania area. The City of Philadelphia has a record high number (544) of murders in 2021. We are tasked with presenting a proposed integrated Healthcare program targeting the population I just mentioned please complete the first 3 steps of this project.
